I spelet följer du Deacon St. John, en biker med MC-väst och kepsen bakochfram. Två år har gått sedan en epidemi slog ut en stor del av den mänskliga befolkningen och kvar finns människor som grupperat sig i läger, och freakers – en sorts zombie som också tenderar att gruppera sig, i horder. Deacon och hans polare Boozer har överlevt i den nya världen genom att ta arbete som prisjägare. En dag får dem ny information om Deacons fru och reser iväg för att ta reda på vad som har hänt. På vägen får man bekanta sig med världen. Open world, med vädersystem och dag/nattcykel. Sök igenom bilar och hus efter ammunition, bensin, delar och tillverka nya vapen som är mer effektiva. Använd omgivningen för att hantera dina fiender på olika sätt. Sätt fällor och lura grupper av fiender in i varandra för att undvika att slösa ammunition.
